How AccordantCare Can Help
AccordantCare is for EHP members or dependents with an autoimmune or genetic condition, epilepsy, or other complex or rare condition (see below for a full list). Once you sign up, you’ll have an AccordantCare nurse and a resource specialist who can help you get answers and learn more about your health, set goals and stay on track, and spot risks early, before they affect your health. AccordantCare will also help you find support in your community, make lifestyle changes for better health, and get the care you need by working with your doctor and health care team
Whether you need a lot of support, or you just have a question now and then, AccordantCare is tailored to your needs. Along with one-on-one phone support, you’ll have digital tools to help you keep track of your health.

The 2022 EHP AccordantCare Incentive Program is Underway!
You, or a member of your household, are eligible to participate in the AccordantCare program for complex conditions. A complex health condition, such as Lupus or Crohn’s Disease, can sometimes need extra care and support. This program gives you 24/7 access to an on-call team of specially trained nurses and health care professionals who specialize in your condition. They can answer questions and help you get the care and support you need, and there is no extra cost.
EHP is now offering an incentive to manage complex conditions through AccordantCare. If you meet the following requirements for the program year you will receive $300 for participating in the program:
• Employee Health Plan member
• Actively participate in at least one AccordantCare program (self-directed members are excluded)
• Complete three of the four AccordantCare nurse facilitated quarterly assessments
• Follow your physician prescribed medication treatment plan
A program year runs January through December. Spouses and dependents of EHP members eligible to participate in the AccordantCare program are also entitled to receive the incentive. Receipt submission is not necessary to receive the incentive.
Exclusions and notes:
Retirees and Cobra members are not eligible to participate in the AccordantCare program. Self-directed AccordantCare members are not eligible for an incentive. Incentives will be disbursed during the first quarter following the completed program year. Participation in multiple programs will not multiply the incentive. Medication adherence is defined as following your physician prescribed treatment plan. If your treatment plan changes, please advise your AccordantCare nurse during your assessment.

With Condition Advisor, a new mobile app, you can access the support and tools of the AccordantCare™ program for complex conditions on your terms. Now it’s easier than ever to stay on track anytime, anywhere
