Employee Health Plan offers a high quality network of providers and facilities.
The provider Tier you select determines the amount of coverage you will receive. To receive maximum coverage, you must use Tier 1 Providers. It is your responsibility to verify and obtain the most current Tier participation each time services are obtained. Finding a provider is simple. Follow the directions below for your specific plan.
Cleveland and Akron General Members:
Use the provider search below to find a Tier 1 provider by name, or perform an advanced search by location in proximity to you. Although Akron Children's may appear in the search, Akron Children's Hospital and their employed physicians are only included in Tier 1 for Akron General members.
Florida Members:
Using these directions, register on UMR. Once registered, you will have access to your Cleveland Clinic Florida specific Tier 1 network. This network includes Cleveland Clinic Florida providers as well the Cleveland Clinic Quality Alliance network in Cleveland. You will also have access to the Tier 2 UMR United Healthcare Choice Plus network. This guide will make searching for a provider even easier.
Nevada and Out of Area Members:
Using these directions, register on UMR. Once registered, you will have access to UMR’s United Healthcare Choice Plus network, which is your provider network. This network also includes the Cleveland Clinic Tier 1 Network of Providers in Cleveland and Florida. There is not a Tier 2 network for OOA.
